Troop Roster and Equipment Planner

• The troop roster and equipment planner forms can be completed online through the Summer Camp Management System located here: https://www.northernstarbsa.org/scms/.

Medical Forms

Everyone attending camp will need to have a medical form completed prior to arrival. There are 2 health forms to choose from, please select the one that applies to your situation:

• Health form for any participant of any age staying at camp for 72 hours or longer (doctor's signature required)
  • Annual health form for long-term stay
• Health form for Webelos Scouts or any participant of any age staying at camp for less than 72 hours (no doctor's signature required)
  • Annual health form for short-term stay
